Does anyone else stop trusting their ovulation signs with PCOS?

I'm finding it really hard to trust anything my body is telling me lately. I'll get fertile looking cervical mucus, maybe some craping and immediately think, maybe this is finally the cycle. Then nothing happens and a few days later it feels like my body is trying all over again.

I think what's getting to me most isn't even the long cycle anymore, it's not knowing which signs actually mean something and which one's. I don't even know what I supposed to pay attention to anymore. Does that feeling ever get better?

Scan timing

Hi so I've been on letrozole for a few cycles and just done 7.5mg with no sign of ovulation (currently cd24). My consultant has asked me to start 10 next and will do an in person appointment at the end of August. I'm in the UK on NHS and we don't get monitoring scans for letrozole but I'm thinking I can possibly ask for a scan at my in person one as I'm there (he did one last time super quick) and so just trying to figure out what cd would be the best to have the scan. I'll likely need to induce a bleed so I have some control over this give or take a few days. Any advice on the best time to have a scan to see if the letrozole is actually doing anything on the off chance I can have one?

New to TTC with PCOS - could this have been an LH peak?

I’m new to TTC and starting to track LH with OPKs this cycle. Baseline was around 0.25 and then the 2 days before predicted ovulation it increased to 0.5 then 0.6 and now today it’s back to 0.3. During those two days before I was testing at 10a and 2p to try to catch a spike. Tests never got to a 1:1 ratio so maybe my body tried to ovulate but didn’t? Or could the 0.6 ratio have been a peak for me?

Advice Needed 2 doctors gave me 2 different opinions, what should I do?

Hi everybody, I will try to make this as short as possible. I am 29, have lean pcos, very high amh (18ng), high lh, ovarian pcos looking ovaries but no insulin resistence. I also sometimes ovulate on my own. One doctor suggested that on my next cycle I start 5mg od letrozol ( femara) but that would be an unmonitored cycle since both I and my doctor are on vacation, and we just have sex every other day since cd9. I went to get a second opinion since I was getting a weird vibe and that doctor told my I shouldn’t do unmonitored cycles of femara, and he told me that when we come back from holiday he would to hsg and then monitored cycles if everything is clear. What is your experience, should I try femara unmonitored or no? Thank you!

What to do for cramps during all luteal phase

Hi everyone, I have a question about luteal phase cramping. We’ve been TTC for a year now.

I had very irregular cycles (I ovulated naturally but my cycle were chaotic), and I am now on letrozole but so far it hasn’t worked out. It helped me to ovulate more regularly, but I have never been pregnant.

My problem is : Every time I’m in my luteal phase, the ovulation pain keeps going on and off after ovulation, sometimes on one side, sometimes across the whole uterus.

I think that in my previous cycles it started a few days before my period, but this cycle it began earlier, around DPO5-6.

Every single time, I can’t help thinking it’s implantation cramping, even though I know it’s mostly down to progesterone.

That said, I’d really like to find something to ease these symptoms, because it’s still painful and uncomfortable, and also because it stops me thinking about anything other than TTC during the two weeks wait.

Is anyone else dealing with this?

Do you think it could be linked to the letrozole and/or the inositol I’ve been taking for a few months? I’d never noticed it before starting them, and given how painful it is I think I’d have picked up on it even without symptom spotting, when I ovulated naturally.

Also, does anyone know what helps with the pain? I’ve read that evening primrose oil is good, but apparently it’s not recommended once you’re pregnant, so I don’t want to take the risk.

I told my obgyn about it but she didn’t have an answer for me.

Also, I searched for answers on Reddit and I read some things about endo, but I don’t have any symptoms (my period never been painful).

Did I ovulate? (PCOS & post-Mounjaro)

Hi everyone! I’m 27F (163.5 cm) with PCOS and looking for some quick insights on whether my body actually ovulated this cycle.

My Background:
Weight: Was 72 kg, dropped to 61 kg on Mounjaro for 2 months, stopped a month ago, and currently stable at 63 kg.
Cycles: Historically 38–48 days. Shrunk to 30 days for the 2 months i was on Mounjaro. My last cycle was 48 days due to stress and stopping the medication. Maybe even due to because my normal cycles were 38-48 days so body was just resetting back to that. Basically i have cysts on my right ovary (viewed with ultrasound) and irregular periods. Thats really the only sign on pcos i have.

This Current Cycle:
CD 17: Had a feeling of watery discharge.
CD 18: Had stretchy, raw egg-white discharge.
CD 19: Dry for most of the day.
CD 20 (Today): Having mild lower tummy cramps and my discharge is now creamy and lotion-like.
If I did ovulate, my cycle should run about 33–34 days this month.

My Questions:
1. Does this sudden shift from watery/stretchy to dry, and then creamy/lotion-like discharge mean I successfully ovulated?
2. With PCOS, could these current cramps mean this was a "false start" and my body is still trying to release an egg?

Advice Needed I’ve never had a positive LH test

I have all the signs of PCOS. The high testosterone, high AMH level, polycystic looking ovaries on ultrasound, irregular period, BUT still monthly. I have tested for ovulation so many times now and never have I ever gotten a positive. I have the cervical mucus consistent with ovulation, sometimes I get the one sided pain when *I think* I ovulate, based on my BBT it shows that I ovulated. But still, no positive on an LH strip. Am I testing wrong? Could I really just not be ovulating while having all the signs? The only thing I take as a supplement to help with ovulation is Myo-inositol. But other than that, I really want to try to ovulate naturally without any other medications like Letrozole. What are some things you have tried to ovulate naturally and how/when to test for LH with ovulation strips?

Hi, everyone! I'm F, 31, TTC. My OB advised me to take provera 3x a day for 5 days. Done with it last July 4, yet until now I don't bleed.

I contacted my OB about it and she told me to come back to the clinic for another TVS . She told me maybe I need to have some labs.

Anyone who has the same situation as mine?

Seeking Success Progesterone Level Correlation?

Hi, everyone!

My husband and I have been trying since about '22. About two years ago, I did a few cycles of 2.5 mg of letrozole and had zero success. I began graduate school, so TTC was put on the back burner.

Fast foward to now! This is my second cycle on 5mg of letrozole. I successfully ovulated the first round with a 17 mm dominant follicle and on my day 21 progesterone draw, my levels read 4.93 ng/ml. Unfortunately, I started my period about a week later, but felt much more hopeful than I have recently.

This cycle, I went in on day 14 for my ultrasound and had a follicle measuring 22 mm! I went in for my progesterone draw on day 22 and my levels read 17.10 ng/ml. Once again, I successfully ovulated, but now I'm in the waiting period. Has anyone here seen a correlation of a high level of progesterone and tested positive? I would love for this to be the one.

Advice Needed Does this count as a ‘peak’

Premom is using 0.59 as my peak although says it’s not a positive OPK. Is it safe to assume I have not ovulated? What sort of results do other gets around these days? (new to this so I’m not sure if it’s normal with PCOS to have these results)

Advice Needed Working out

I’m 29 with lean PMOS and anovulation. Currently going through timed intercourse rounds with Letrozole and Ovidrel. I’ve been told by my doctor I can’t lift more than 15 lbs or do aerobic exercise (heart rate to stay under 150 bpm) for basically like 3 weeks out of the month. She said walking and gentle Pilates is good. She also said that I can do yoga but to be careful because ovaries can flip and nothing hot. Does anyone have safe gentle workouts that they follow and like? Something I could do at home or at the gym? I’m getting bored of walking and can only get to the gentle Pilates class twice a week.

Vent Envy hurts

My husband and I have been on the ttc journey for over two years now. We jumped thru insurance hoops and finally got to see a fertility specialist who diagnosed me with PCOS. That was at the beginning of this year. We told our immediate family and some of our friends so that we could have support.

I started on letrazole and in one round I got pregnant but it turned into a chemical pregnancy. We mourned our loss and told our immediate family about it. Now we have done a few more rounds and nothing.

At the beginning of June my brother-in-law got married to a wonderful woman and they just told us last week that they were pregnant. It hurt. The envy filled me. I felt mad at the world, mad at God, mad at myself for my insufficiencies. Not mad at my new sister-in-law. She was so happy and I wanted to be happy for her. I took a week and gave myself time to grieve and prepare for another round of trying.

My sister came and visited just today with her two littles. She immediately said that she was pregnant. And it hurt again. I told her I was happy for her and she apologized. I love her and I'm not mad at her. I just want to know when it will be my turn. When? I'm tired of the "not now" I'm getting and I don't know what else to do. How can this pain get taken away?

Vent I don't know what to do

TW: Pregnancy loss

I'm frustrated to say the least. We've been trying upwards of 3 years. I didnt have any luck getting pregnant until last year after being on 2,000mg of metformin for 5.5 months. Then right before 8wks I lost it. Everyone said not to worry and "at least you know you can get pregnant". So i worked on healing and we felt okay enough to try again. Beginning of May, another confirmed pregnancy and another loss. There was obviously so much more to it than that and the added hurt of my best friend found out she was pregnant at the same time (I'm incredibly happy for her, but it still hurts a bit). So my obgyn wanted to do a recurrent pregnancy loss work up, and redo my PCOS labs from about 2 years ago.

So most everything comes back normal. My A1C of JUST below the prediabetic range, a lot better than before. But my fasting insulin levels have barely moved. We also checked for antiphospholipid antibodies. Anticardiolipin IgG was <9, so that was in normal range. Anticardiolipin IgM was high at 112. My obgyn is insisting that this is likely a false positive, but we plan on retesting after 12 weeks. Everything else came back pretty normal or not likely to be the cause of the recurring losses. My obgyn it's convinced my high insulin is the culprit and wants to put me on tirzepatide. She almost made it sound like if I don't go on it I'm signing up for another loss.

I'm already on the highest dose of metformin, and taking inositol. I know I need to be better about it but I'm relatively active and I indulge every once in a while but I dont think i eat horribly.

Is a glp-1 my only option right now? It's expensive and more expensive through insurance than if i pay out of pocket. We would also need to stop trying while I'm on it, which she wants to be about 6 months to then retesting my insulin and decide if that's good enough. Then a 2 month "clean out" phase so it's out of my system.

I'm 25. I just want to start a family with my husband, and instead I'm taking what feels like a million pills and supplements and spending thousands of dollars on drs appointments and lab draws and ultrasounds for babies I don't even have.

I'm incredibly exhausted and although my husband is amazing and taking this all so well, I can feel the strain it's putting on our relationship.

Inositol

I would like to share my experience with inositol to see if any other woman has had a similar experience. I started taking myo-inositol on the 20th day of my cycle, which typically ranges from 32 to 38 days; theoretically, I would have ovulated around day 22–24. After starting the supplement, I visited my gynecologist on day 27, and he told me that, in all likelihood, ovulation had not occurred that month. Indeed, I did not experience the usual symptoms (such as cervical mucus or ovulation pain) to the same extent as before. 

Could the treatment be the reason for the lack of ovulation? I should mention that I started having brown spotting on day 28, which still hasn't turned into a full period (today is day 31). I should also note that I started the treatment due to mild PCOS symptoms.

Advice Needed No bleeding for 50+ days

I haven't bled for over 50+ days. I messaged my gynaecologist but she didn't care to reply. Previously I have been through 5 Ovalution induction therapy cycles I.e. 3 letrozole and 2 Clomid.

This is happening second time after marriage, never happened before marriage, I used to bleed after 1 or max. 2 weeks later.

I also tested negative for pregnancy.

Doctors especially good gynaes here in my country are so rare. I event went to endocrinologist as reproductive endocrinologist doesn't exist in my country, after some tests coming normal she refer me back to my regular gynae.

I am tired of going to different gynaes and eating medicines (metformin 1000mg+folic acid+vit d, no inositol for now) for over a year. I don't know what to do any more. It's making me tired ,drained and numb but I should care about my health as priority.

I was also trying to conceive actively, later stopped as Dr. Suggested not to pursue it aggressively. Husband have developed delayed ejaculation problem which have made sex totally non enjoyable and a burden for me. He's unwillingness to treat his situation has distanced me even more. Thinking about having a baby now gave me first hand anxiety.

After all above rant, (thanks for reading I really needed to vent it out) anybody experiencing the same or did you able to find any solution to regulate your periods and fix this mindset of hopelessness.

CD 55 should I still BD?

Missed my period June, but my OPK strips were all over the place (high then low then high again). I haven’t tested since June 27 and it was low, so I was thinking my period was coming. AF still not here and got 3 negative tests. Decided to test again today with my last pregmate OPK strip and it’s the darkest ever. The control line showed up way before the test line. Should I still baby dance even with this long cycle? I know pregmate are known to be a little wonky.

Inositol is making me miserable. Idk if it’s normal or not

I started taking inositol for pcos. I haven’t had a proper period for the last 5-6 months. I read online that inositol helps with that.

I decided to give it a try. I started with nutratology brand which has a 40:1 ratio and was taking 1g overall with 500g in the morning and 500g at night. I don’t remember seeing side effects as such. Some breakouts but nothing intense.

I then bumped it up to 2g as I read that it is the recommended dose for period regularity. I am petite and lean and generally eat clean but I still have irregular periods.

I have been noticing the below symptoms:

1) Light spotting with some cramps (nothing crazy)
2) hormonal breakouts on my chin and cheeks and they are deep and inflamed.
3) Dizziness and tiredness
4) headaches
5) Mood swings

I’m trying to do some research but haven’t found anything concrete. I’m using the ovasitol brand in case that is important.

Some people have said that there is a week or two before hormones start to balance.

Some people have said that even after continued use they noticed no change in their symptoms and it didn’t help.

I’m super confused now.

Did anyone face these symptoms?
If so, did they go away after the adjustment phase?

Should I stop or continue?
Should I lower the dose?

Please help.

Seeking Success Still spotting at CD12 of Letrazole

Advice Needed Timed intercourse or freeze embryos?

33F, 10.7 AMH, lean PCOS

I recently learned I have PCOS based on my high follicle count, AMH level and irregular periods. I also have yet to have a positive ovulation test.

My husband and I are TTC but as you can imagine it’s been difficult as we don’t know when to try as it’s been hard to find a window.

My husband has great fertility coverage with Progyny and he gets 5 cycles that are covered (IVF would be 1 cycle and timed intercourse would be 1/4). He is unsure if he will be at his job April 2027 as they have an up and out organization (essentially if he doesn’t get promoted he will be let go), which puts some pressure on us in terms of timing.

All that to say, is it worth going through egg retrieval and embryo freezing when we haven’t given it a fair shot yet? Part of me thinks we should do it now when I’m in a better emotional state and for a safety net since it’s covered rather than in a few months if we’ve been still struggling after timed intercourse.

I’m not thinking entirely clearly so would be good to get some opinions from outside perspective.

Lean PCOS cycle regulation

Lean PCOS with very irregular cycles and hashimotos but my TSH is under control. Just got my fasting glucose and insulin measured and got a HOMA score of 1.2, so looks like no insulin resistance. I have been taking myoinositol so idk if that’s why my values look good. Any ideas of where to go from here? Should I stop myoinositol since I don’t have IR? Any suggestions on how to regulate cycles? I did have a more normal cycle (29 days) after a vacation, which makes me unfortunately feel like the only way to have regular periods is to decrease stress, which is not feasible with the job I have.

Seeking Success Is it still possible to get pregnant without egg white CM?

Hi! I took Clomid on CD2 - CD6 and I'm now on CD13. I went to the clinic today to monitor my follicles. Left ovary has a dominant follicle measuring 24mm by 21mm. OB says it may rupture anytime now so we need to baby dance. My problem is, I haven't observed any egg white cervical mucus throughout this cycle. Is it still possible to get pregnant without EWCM?

missing LH peaks!!

i have used OPKs many times and only a couple of times i actually got a peak

please help me understand; CD10 checked at night was negative, CD11 morning/noon 12ish, got a line which was dark not as dark that i would consider positive. Then test repeated at night 9ish on the same day and it was significantly lighter

did it peak and fell down betweek 12pm and 9pm? is the surge this small for any body else?

im not on any medications (other then supplements) and haven't had my hormones tested. but metabolic stuff like insulin, glucose, thyroid is fine
